Release of Updated Uniform DRAI Forms
A fully updated Birth Mother Uniform Donor Risk Assessment Interview (DRAI) has been released for use when the donor is less than 18 months old or has been breastfed in the past 12 months. This version includes question 22 about Ebola virus infection and three questions (30-32) intended to collect additional information about risk of CJD in the event that the donor did not leave the hospital and a Child Donor DRAI was not completed. These questions were added to ensure compliance with FDA’s donor eligibility criteria and are consistent with those in other UDRAIs.
Editorial changes have been made to the Uniform DRAI for Donor Greater than 12 years old. The note regarding international travel or residency after question 24c was revised to reference question 11 on vaccinations and the reference in question 18 to questions 18a. to 18g. has been changed to “18a. to 18f.”. The Eye-Only DRAI for Child Donor ?12 years old question 29 was edited to reference 29h. in the Note to Interviewer section.
