Speaker agreements*

As a speaker for the Cornea and Eye Banking Forum, I agree to each of the following:

I will submit my presentation by the due date.

I will only include material and content that I have permission to use and will cite material from outside sources.

The presentation will be free of any commercial bias for or against any product and I will include a slide with my relevant financial disclosures.

I grant EBAA and Cornea Society a non-exclusive, world-wide, royalty-free license to record, broadcast, and sell or offer as complimentary my Presentation.

I grant EBAA and Cornea Society the right to edit, transcribe, distribute and sell or offer as complimentary, at EBAA and Cornea Society’s discretion, recordings and copies of my Presentation and Materials, in whole or in part, on any EBAA and Cornea Society websites or in any and all media now existing or hereafter developed by EBAA and Cornea Society throughout the world.

I grant EBAA and Cornea Society the right to use Presenter’s name and voice, Presentation title, Presentation description and biographical information in publicity, promotion and advertising in connection with the Presentation and the Materials.

I accept the invitation to present at the 2024 Cornea and Eye Banking Forum on October 18.
