CEBF Speaker Agreement

As a presenter, I confirm each of the following:
• I will submit my completed slide deck by 11:59 p.m. Eastern on Wednesday, September 23, 2026.
• I will include a financial disclosure slide immediately following my title slide, listing every financial relationship I have had with a commercial company within the past 24 months — naming the company and the nature of the relationship — or stating that I have no financial interest to disclose. I understand my disclosures will be reviewed prior to the Forum and that unresolved conflicts of interest may disqualify my presentation from credit.
• I will keep my presentation within the time allotted to me, including any question-and-answer period.
• I understand that I am responsible for my own Forum registration and travel expenses.
• If I become unable to present, I will notify EBAA as soon as possible so that alternative arrangements can be made.

• I will submit a PDF of my completed poster by 11:59 p.m. Eastern on Wednesday, September 23, 2026. I understand this PDF will be made available to attendees through the meeting app.
• I will include a financial disclosure statement on my poster and in the PDF I submit, listing every financial relationship each author has had with a commercial company within the past 24 months — naming the company and the nature of the relationship — or stating that the authors have no financial interest to disclose. I understand my disclosures will be reviewed prior to the Forum and that unresolved conflicts of interest may disqualify my presentation from credit.
• I will be present at my poster board during the poster session on Friday, October 9, 2026, from 3:15 to 3:50 p.m.
• I am responsible for printing and transporting my own poster, and I understand EBAA does not print posters or receive them on site.
• I understand that I am responsible for my own Forum registration and travel expenses.
• If I become unable to present, I will notify EBAA as soon as possible so that alternative arrangements can be made.

As a presenter at the 2026 Cornea and Eye Banking Forum (the “Forum”), presented by the Eye Bank Association of America (“EBAA”) and the Cornea Society, I agree to each of the following. In this agreement, “Presentation” means my oral presentation at the Forum together with the slides, handouts, and any other materials I prepare or deliver in connection with it.
1. I will include only material and content that I own or have permission to use, and I will cite material drawn from outside sources. I represent that my Presentation does not infringe the copyright, trademark, or other rights of any third party.
2. Any patient images, case details, or other patient information in my Presentation are de-identified in accordance with HIPAA, or are used with the patient’s written authorization.
3. My Presentation will be free of commercial bias for or against any product, service, or company. I will refer to products by generic name wherever possible, and any recommendations I make will be based on the best available evidence.
4. I grant EBAA and the Cornea Society a non-exclusive, worldwide, royalty-free, perpetual license to record, reproduce, edit, transcribe, broadcast, display, and distribute my Presentation, in whole or in part, and to sell it or offer it at no charge, at EBAA’s and the Cornea Society’s discretion, through their own channels or those of their partners, on any EBAA or Cornea Society website and in any and all media now existing or hereafter developed, throughout the world.
5. I grant EBAA and the Cornea Society the right to use my name, voice, image, and likeness, along with my Presentation title, Presentation description, and biographical information, in publicity, promotion, and advertising in connection with the Forum and my Presentation.
