Committee Confidentiality Agreement Form

As a member of EBAA’s Accreditation Board, I hereby pledge that:

• I will respect the confidential information and intellectual property of all eye banks with which I interact
•I will not disclose confidential information or intellectual property to anyone, except in the course of my service on the Accreditation Board
•I will not use the knowledge I obtain during an accreditation inspection to replicate the policies, processes or products of the inspected eye bank without the express permission of the CEO of that eye bank
•I will protect the confidential information of the eye banks I inspect with at least the same care I take in protecting my own eye bank’s information

Furthermore, I pledge the following:
•I will fairly review and evaluate the banks I inspect without bias or consideration of the bank’s relationship to my bank or to others
•I will review all materials submitted by the inspected eye bank prior to my arrival on-site
•While on-site, I will dedicate my full time and attention to the inspection
•I will review, understand and faithfully interpret EBAA’s Medical Standards and other guiding documents
• I will contact the Accreditation Board Chairs or EBAA staff if I have questions about the interpretation or intent of the Medical Standards or other guiding documents
•I will maintain the confidentiality of all Accreditation Board proceedings before, during and after its meetings
•I will disclose any conflicts of interest for potential inspection assignments
